How to Make an Appointment
Step by step
- Call 0-2206-2900 Ext. 10457 during clinic hours to
book in advance. - New patients: via internal consultations only
- On the day, bring your ID, medical records, a list of current
medications, and any imaging results you have. - Please arrive about 30 minutes before your appointment to register.
For referring physicians: we welcome consultations for patients with chronic pain or cancer-related pain via the hospital’s internal referral system.
